All applications must be accompanied by copies of the P4, P5 (Mid-Year and Year-end) and P6 (Mid-Year) results, a copy of Parent's IC and a Letter of Recommendation from the Primary School.

You may submit the completed application form with the supporting documents personally to the school at the address below:

(You may also choose to submit the Application Form with all supporting documentsvia mailto the same address as above. You should receive the DSA Submission Receipt (with an acknowledgement number accompanied by a school stamp beside it) from the school via mail. Please give the school a call should you fail to receive the mailed DSA Submission Receipt 2 weeks after you have mailed the original Application Form.)

Upon submission of your application form, you will be given a registration number. The general office will call you for the arrangement of the trial session and the interview if you have passed the trial. You can use this registration number to check if you have been shortlisted for trials and interviews by calling our general office at 6288 9382.

The closing date for submission is at 5pm on 27 July 2012. Incomplete applications without supporting documents will not be considered.
